Based in Hollis, NH, Cancer Transitions is dedicated to empowering cancer survivors as they navigate life after treatment. Our evidence-based program is specifically designed for individuals who have completed their cancer treatment within the last 24 months, although those further along in their recovery journey are also welcome to benefit from our resources. We provide essential support through tailored nutritional guidance, customized exercise plans, emotional health education, and comprehensive medical management education. Our mission is to ensure that every survivor has access to the tools and knowledge needed for a fulfilling and healthy life after cancer.
