Located in Windham, NH, Cancer Transitions is dedicated to supporting cancer survivors who have recently completed their treatment. This evidence-based program focuses on individuals within 24 months of finishing their treatment, although any survivor is welcome to participate at any stage of their journey. We provide essential resources that include nutritional support, tailored exercise plans, emotional health education, and guidance on medical management. Our mission is to empower survivors with the tools and knowledge they need to thrive and navigate their path to recovery.
