Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors in Caldwell, NJ, who may be at risk of nursing home placement. Our mission is to enhance the quality of life for older adults by offering vital in-home services that complement the care provided by their families and friends. We provide a comprehensive range of services including respite care, home health aides, home modifications for better accessibility, personal emergency response systems, home-delivered meal services, social adult day care, adult day health services, special medical equipment and supplies, transportation, and chore services.
