Jersey Assistance for Community Caregiving (JACC) is dedicated to empowering individuals in Clementon, NJ, who are at risk of needing nursing facility care but wish to remain in the comfort of their own homes. We offer a comprehensive range of in-home services tailored to meet the unique needs of each participant, focusing on prevention and support. Our services include respite care, homemaker services, home modifications to enhance accessibility, personal emergency response systems, home-delivered meals, social adult day care, and transportation, among others. JACC serves those who do not qualify for Medicaid or its waiver services, with a commitment to affordability through a sliding scale co-pay based on income. Supported entirely by state funds, JACC is passionate about enhancing the quality of life for individuals in our community.
