Based in Elizabeth, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to enhancing the quality of life for seniors at risk of transitioning to nursing facilities. With a compassionate approach, JACC offers a variety of in-home services that complement the support provided by family, friends, and neighbors. Our mission is to empower seniors to remain in their homes and thrive independently through essential services such as respite care, home health aides, environmental accessibility modifications, personal emergency response systems, home-delivered meals, social adult day care, adult day health services, specialized medical equipment, transportation, and chore services. We understand the urgent need for comprehensive care and are here to provide practical, supportive solutions that make a difference in the lives of our seniors and their caregivers.
