At Jersey Assistance for Community Caregiving (JACC) in Elizabeth, NJ, we are dedicated to empowering individuals at risk of institutional placement to remain in the comfort of their own homes. Our state-funded program offers a comprehensive range of in-home services tailored to meet the unique needs of each participant, including care management, home health aides, respite care, home modifications, specialized medical equipment, chore services, personal emergency response systems, home-delivered meals, and both caregiver and recipient training. We understand that every individual's situation is different, which is why we provide valuable resources, such as social adult day care and attendant training, to enhance quality of life. While participants may contribute to the cost through a sliding scale co-pay, our mission is to ensure that financial considerations do not prevent anyone from accessing essential care in their community.
