Our mission is to connect people to essential care as early as possible, ensuring their health and reducing the potential for transmission to others. We offer a comprehensive range of free and confidential services, including HIV counseling and testing, early intervention, and access to an on-site infectious disease physician. Our program prioritizes individualized medical case management and provides crucial support, such as dental and nutrition counseling, behavioral health services, and transportation assistance. We also facilitate outreach and community health education, linking patients with social services to address housing, financial concerns, substance use, and legal needs.
