Jersey Assistance for Community Caregiving (JACC) is dedicated to supporting individuals in Linden, NJ, who are at risk of needing nursing facility care, offering a comprehensive range of in-home services tailored to help them thrive in their own homes. Our mission focuses on providing essential care management, home health aides, respite care, and assistance with necessary home modifications and medical equipment. We also offer chore services, personal emergency response systems, home-delivered meals, and specialized training for both caregivers and recipients. By empowering our participants with these resources, we aim to prevent unnecessary relocations to nursing facilities, all while working to keep care affordable through a sliding-scale co-pay model.
