Located in Linden, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors at risk of nursing facility placement. Our mission is to enhance the quality of life for older adults by providing essential in-home services that complement the care given by family, friends, and neighbors. We offer a comprehensive range of services, including respite care, home health aides, environmental accessibility adaptations, personal emergency response systems (PERS), home-delivered meal service, social adult day care, adult day health services, special medical equipment and supplies, transportation, and chore services. At JACC, we believe in empowering seniors to remain in their homes safely and comfortably while ensuring they receive the support they need.
