Jersey Assistance for Community Caregiving (JACC), located in Monroe Township, NJ, is dedicated to enhancing the quality of life for seniors at risk of nursing home placement. We offer a comprehensive suite of in-home services that support both seniors and their caregivers, ensuring that individuals receive the assistance they need to remain safely in their own homes. Our services include care management, home health aides, respite care, home modifications, and provision of special medical equipment and supplies, in addition to chore services, personal emergency response systems, and home-delivered meals. We also provide social adult day care and adult day health care, coupled with transportation options to meet our clients’ needs. JACC operates with a sliding co-pay fee structure based on income, making our services accessible to those who require them most.
