The Jersey Assistance to Community Caregivers Program (JACC) is dedicated to enhancing the quality of life for seniors in Monroe Township, NJ, who are at risk of being placed in nursing facilities. Our mission is to provide a comprehensive range of in-home services that support and empower both seniors and their caregiver networks, including family, friends, and neighbors. JACC offers essential services such as respite care, home health aide support, environmental accessibility adaptations, personal emergency response systems, home-delivered meal services, social adult day care, adult day health services, special medical equipment and supplies, transportation assistance, and chore services.
