Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors in Montclair, NJ, who are at risk of needing nursing home placement. We offer a comprehensive range of in-home services designed to enhance the capabilities of their existing caregiver networks—whether family, friends, or neighbors. Our program includes essential services such as respite care, home health aide support, environmental accessibility adaptations for safe living, personal emergency response systems, and home-delivered meal service. Additionally, we provide social adult day care, adult day health services, access to special medical equipment and supplies, transportation assistance, and chore services, all aimed at promoting independence and quality of life for our valued seniors.
