Located in Neptune, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors at risk of nursing facility placement by providing vital in-home services. Our mission is to empower seniors and their families by offering a comprehensive range of services that enhance the care provided by family, friends, and neighbors. JACC's offerings include respite care for caregivers, home health aide services, environmental accessibility adaptations, personal emergency response systems, home-delivered meal services, social adult day care, and adult day health services. Additionally, we provide access to special medical equipment, supplies, transportation, and chore services, ensuring that seniors can maintain their independence and quality of life in the comfort of their own homes.
