The Middlesex County Multi-Assistance Program (MCMAP) in New Brunswick, NJ, is dedicated to providing essential support services to homebound frail and elderly individuals, as well as disabled adults aged 18 to 64. Our mission is to help these residents remain safely in their communities and avoid premature nursing home placements. MCMAP offers a range of critical services, including prescription assistance, home health care, social and medical adult day care, home-delivered meals, medical equipment and supplies, and barrier-free assistance through home and environmental modifications. Our compassionate care managers work collaboratively with clients to develop personalized care plans that ensure the delivery of appropriate services. While all clients are required to contribute a cost share based on income, we strive to make our support accessible to those who need it most.
