The Middlesex County Multi-Assistance Program (MCMAP) in Old Bridge, NJ, is dedicated to supporting homebound frail elderly and disabled adults aged 18 to 64 by offering essential services that enable them to live safely and independently in their community. Our program serves as a critical 'safety net' to prevent unnecessary nursing home placements, providing a range of services including prescription assistance, home health care, social and medical adult day care, home-delivered meals, medical equipment and supplies, and barrier-free home modifications. MCMAP's compassionate care managers work closely with clients to create personalized care plans and ensure that each individual receives the appropriate services tailored to their needs.
