Jersey Assistance for Community Caregiving (JACC) is a vital state-funded initiative located in Rahway, NJ, dedicated to providing essential in-home services that empower individuals at risk of nursing facility placement to stay in their own homes. Our comprehensive programs include care management, home health aides, respite care, home modifications, special medical equipment, chore services, personal emergency response systems, home-delivered meals, caregiver and recipient training, and social adult day care. We are committed to tailoring our support packages to meet individual needs, thus delaying or preventing institutionalization.
