Located in Riverside, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors at risk of nursing home placement by offering a range of essential in-home services. Our compassionate approach complements the efforts of family members and friends, ensuring that each individual receives the care and assistance they need in the comfort of their own home. JACC provides various vital services, including respite care, home health aide support, home modifications for improved accessibility, personal emergency response systems, and nutritious home-delivered meal services. We also offer social adult day care, adult day health services, necessary medical equipment and supplies, transportation assistance, and chore services.
