Based in Union, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors at risk of nursing home placement, helping them maintain their independence and quality of life in the comfort of their own homes. We offer a comprehensive range of in-home services that enhance the support provided by family, friends, and neighbors. Our programs include respite care, home health aides, environmental accessibility adaptations, personal emergency response systems (PERS), home-delivered meal services, social adult day care, adult day health services, special medical equipment and supplies, transportation, and chore services. JACC is committed to providing practical, compassionate assistance tailored to the unique needs of each senior, ensuring that they receive the care and support necessary to thrive in their community.
