Cancer Transitions is a supportive program based in Bernalillo, NM, dedicated to empowering survivors of any cancer diagnosis who have recently completed treatment. While primarily designed for those who have finished their treatment within the last 24 months, our resources can be beneficial at any stage of survivorship. We offer essential services including nutritional support, exercise customization, emotional health education, and guidance in medical management. Our mission is to provide holistic support that fosters resilience and enhances the quality of life for cancer survivors as they move forward on their healing journey.
