Cancer Transitions is a compassionate program based in Chimayo, NM, dedicated to supporting survivors of all cancer diagnoses who have completed treatment. While specifically designed for those who have finished their treatment within the last 24 months, our resources are valuable for survivors at any stage of their journey. We provide essential services including nutritional support, personalized exercise programs, emotional health education, and medical management guidance. Our mission is to empower cancer survivors with the tools they need to thrive, fostering resilience and well-being as they navigate life after cancer.
