Located in Church Rock, NM, Cancer Transitions is dedicated to empowering cancer survivors who have recently completed their treatment. This evidence-based program serves individuals within 24 months post-treatment, while also welcoming survivors at any stage of their journey. We focus on enhancing the quality of life for survivors through vital resources, including nutritional support, personalized exercise plans, and education on emotional and medical health management.
