The Living with Type 2 Diabetes Program, located in Raton, NM, is designed to support individuals recently diagnosed with type 2 diabetes by providing essential education and resources over the course of 12 months, free of charge. Our program offers six informative e-booklets, each tailored to help participants understand and manage their condition effectively. We communicate in both English and Spanish to ensure accessibility for everyone. Participants can also benefit from a monthly e-newsletter filled with seasonal tips, local community event information, and diabetes management strategies. Additionally, enrollees who share their phone number have the opportunity to join our monthly 'Ask the Experts' Q&A series, where they can engage with ADA diabetes experts, explore the relationship between diabetes and heart health, and access valuable giveaways that promote thriving health with diabetes. We are committed to empowering individuals with the knowledge and support they need to navigate their journey with type 2 diabetes.
