Located in Thoreau, NM, the Living with Type 2 Diabetes Program is dedicated to supporting individuals recently diagnosed with type 2 diabetes through a comprehensive, free 12-month journey of education and management. Designed to meet participants at their unique stage, this program offers six informative e-booklets addressing key topics essential for understanding diabetes, along with a monthly e-newsletter filled with practical seasonal tips and community resources, available in both English and Spanish. Enrollees are encouraged to submit their contact information to join our engaging monthly 'Ask the Experts' Q&A series, where they can connect with ADA experts, ask vital questions related to diabetes management and heart health, and access helpful giveaways to enhance their diabetes experience. Together, we aim to empower individuals to thrive while navigating their diabetes journey.
