Cancer Transitions is an evidence-based program in Bend, OR, dedicated to supporting cancer survivors who have recently completed treatment. Designed primarily for those who finished their treatment within the last 24 months, our program offers essential resources that enhance well-being and quality of life at any stage of survivorship. We provide nutritional support to help individuals rebuild their strength, exercise customization tailored to personal abilities and goals, emotional health education, and essential medical management guidance. Our mission is to empower cancer survivors with the tools and knowledge needed to thrive in their journey ahead.
