Oregon Project Independence (OPI) is dedicated to supporting individuals in Creswell, OR, who are over 60, living with dementia, or facing disabilities, enabling them to maintain their independence and safety at home. Our comprehensive program offers essential services such as personal care assistance, home care, chore services, assistive technology devices, adult day care, respite care, skilled nursing care, and home-delivered meals. We believe in equitable access, so all participants contribute to service costs based on a sliding scale tailored to their income and medical expenses. When you request our services, a trained staff member from the Area Agency on Aging will reach out to schedule a home visit, where they’ll conduct a needs assessment to determine eligibility. If eligible, you’ll receive personalized assistance to create a care plan that fits your unique needs, helping you stay connected and supported in your own home.
