Oregon Project Independence (OPI) is a dedicated initiative based in Grants Pass, OR, aimed at empowering seniors and individuals with disabilities to live independently and safely in their homes. Targeting those aged 60 and over, as well as those living with dementia and certain disabilities, OPI offers a wide range of supportive services including personal care assistance, home care, chore services, and home-delivered meals. In addition, the program provides access to adult day care, respite care, skilled nursing care, and assistive technology devices. Services are structured around a Sliding Scale Fee Schedule, ensuring that costs are manageable based on a participant's household income and medical expenses. Once you request assistance, a trained staff member from the Area Agency on Aging will conduct a thorough assessment in your home to determine eligibility and collaborate with you in creating a personalized care plan tailored to meet your needs.
