Located in Gresham, OR, Cancer Transitions is dedicated to empowering cancer survivors through an evidence-based program designed for individuals who have completed their treatment for any cancer diagnosis. While primarily focused on those who have finished treatment within the past 24 months, we encourage participants at any stage of their survivorship to join us. Our program offers comprehensive support, including nutritional guidance, customized exercise plans, emotional health education, and assistance with medical management.
