Cancer Transitions is a dedicated program based in Happy Valley, Oregon, designed to support cancer survivors who have completed their treatment within the last 24 months, although anyone beyond that timeframe can also benefit. Our mission is to empower survivors through comprehensive resources that include nutritional guidance, personalized exercise plans, emotional health education, and medical management support. We understand the unique challenges faced during survivorship and strive to provide a nurturing environment where individuals can regain their strength and enhance their quality of life.
