Oregon Project Independence (OPI) is dedicated to supporting individuals in Lake Oswego, OR, who are over 60, living with dementia, or who may have disabilities in certain counties. Our mission is to empower these individuals to live safely and independently at home, offering a range of essential services including personal care assistance, home care, chore services, assistive technology devices, adult day care, respite care, skilled nursing care, and home-delivered meals. We understand that affordability is key, which is why we implement a Sliding Scale Fee Schedule, ensuring that costs are tailored to your monthly income after medical expenses. Upon requesting services, a trained staff member from the Area Agency on Aging will reach out to set up an in-home assessment, allowing us to understand your unique situation. If eligible, we will work closely with you to develop a personalized plan that addresses your specific needs, fostering independence while ensuring safety.
