Oregon Project Independence (OPI) is dedicated to empowering individuals in Lebanon, OR, aged 60 and older, as well as those living with disabilities or dementia, to maintain their independence in the comfort of their own homes. Our comprehensive services include personal care assistance, home care, chore services, adult day care, respite care, skilled nursing care, and home-delivered meals, all designed to promote a safe and supportive living environment. We believe that everyone deserves to live with dignity and respect, which is why we use a Sliding Scale Fee Schedule to ensure our services are accessible, taking into account each participant's income and medical costs. When you request our assistance, a trained staff member from the Area Agency on Aging will contact you to schedule an in-home assessment that helps us understand your unique needs. If eligible, we will work collaboratively with you to develop a personalized plan that aligns with your goals and enables you to thrive at home.
