The Living with Type 2 Diabetes Program in Sisters, OR, is dedicated to supporting individuals recently diagnosed with type 2 diabetes through free, accessible education and resources over a 12-month period. This program, available in both English and Spanish, empowers participants with six informative e-booklets tailored to their current knowledge and circumstances post-diagnosis, along with a monthly e-newsletter that offers seasonal tips, resources, and information on local community events. Additionally, enrollees who provide their phone numbers can take part in our monthly Ask the Experts Q&A series, where they can pose diabetes-related questions to ADA experts, learn about the critical connection between diabetes and heart health, and engage in community discussions with others facing similar challenges. Our mission is to equip individuals with the knowledge and tools they need to thrive while navigating their diabetes journey.
