Located in White City, Oregon, Cancer Transitions is a dedicated program designed to support cancer survivors who have recently completed treatment. Although primarily aimed at individuals who have finished their treatment within the last 24 months, our resources are beneficial for survivors at any stage of their journey. We offer a holistic approach that includes nutritional support, personalized exercise plans, emotional health education, and medical management assistance. Our mission is to empower survivors by providing the tools and knowledge they need for a healthier, more fulfilling life beyond cancer.
