About Oregon Project Independence (OPI)
Oregon Project Independence (OPI) is dedicated to empowering individuals in Woodburn, OR, who are over 60, living with dementia, or experiencing disabilities to live independently and safely at home. Our range of services includes personal care assistance, home care, chore services, assistive technology, adult day care, respite care, skilled nursing care, and home-delivered meals, all designed to support your unique needs. Understanding that everyone’s financial situation is different, we offer a Sliding Scale Fee Schedule to ensure that costs are manageable based on your monthly household income and medical expenses. When you reach out for assistance, a trained staff member from the Area Agency on Aging will contact you to conduct a needs assessment in the comfort of your own home. This helps us determine your eligibility and collaboratively develop a care plan that works for you within our program parameters. At OPI, our mission is to provide compassionate, effective support that promotes dignity and independence for those we serve.
