Project Transition provides residential mental health treatment for adults aged 18-65, specializing in care for individuals with co-occurring disorders such as substance use, intellectual developmental disorders, and physical health conditions. Their distinctive approach centers on creating therapeutic communities where participants are called "members" rather than clients or patients, emphasizing dignity and community integration. Residents live in apartment complexes designed to facilitate reintegration into broader society while reducing stigma and promoting normalized social expectations. The program employs dialectical behavior therapy (DBT) with individualized, strength-based approaches tailored to each member's unique goals and needs. A dedicated team of approximately 30 professionals, including certified peer and recovery specialists, provides support daily from 7 a.m. to 9 p.m., with emergency assistance available after hours. The structured program includes skills workshops, medication management, individual therapy sessions, psychiatric appointments, group therapy, community meetings, goal planning, and access to recovery support groups like AA and NA. Transportation is provided for off-site appointments and meetings. Project Transition also offers Systems of Support (SOS) for children and adults whose behavioral health concerns impact their community functioning, working collaboratively with crisis teams and family support systems to reduce hospitalizations and prevent crises. The program allows daytime visitors with housemate approval and provides partially furnished apartments where members can bring personal belongings.
