Our mission is to empower participants to take charge of their lives through a holistic approach that combines education, advocacy, and recovery-oriented support. We aim to build essential life skills and foster independence by providing tailored services such as problem resolution, health management, and linkage to preventive resources. With both weekly and monthly contact options available, our committed team works closely with families and community organizations to create individualized care plans, ensuring everyone has the tools they need for a successful recovery journey. Enrollment requires a referral, and while our services are available during regular business hours, we also offer 24/7 emergency support to address urgent needs.
