Based in Monroeville, PA, Cancer Transitions is a compassionate and evidence-based program designed to support cancer survivors in their journey after treatment. Open to individuals who have completed any cancer treatment within the last 24 months, our initiative also welcomes those further along in their survivorship. We are dedicated to providing essential resources that include nutritional support, personalized exercise plans, emotional health education, and medical management guidance. Our mission is to empower survivors with the tools and knowledge needed to thrive in their post-treatment lives, fostering a community of hope and resilience.
