Project Transition provides specialized care for adults ages 18 to 65 experiencing mental health conditions, including those with co-occurring substance use disorders, intellectual developmental disorders, or physical health challenges. Their residential treatment program is uniquely structured around apartment living and guided by a therapeutic community philosophy. Distinguishing themselves in the mental health field, Project Transition refers to participants as "members" rather than clients or patients, emphasizing their belonging to a supportive community. This approach aims to reduce stigma while promoting normalized social expectations and meaningful community integration. Members reside in apartment complexes that serve as stepping stones to broader community reintegration. The program employs dialectical behavior therapy (DBT) with a strength-based, individualized focus. A dedicated team of approximately 30 professionals, including certified peer and recovery specialists, provides comprehensive support from 7 a.m. to 9 p.m. daily, with emergency assistance available after hours. The treatment schedule includes skills workshops, medication management, individual and group therapy sessions twice weekly, community meetings, goal planning, and access to Alcoholics Anonymous and Narcotics Anonymous meetings. Transportation to off-site appointments and support groups is provided by an onsite driver. Project Transition also offers Systems of Support (SOS) for children and adults whose behavioral health concerns impact their community functioning. They collaborate with crisis teams and family support systems to reduce hospitalizations and prevent crises. The program measures success through 13 specific criteria and allows daytime family visits with housemate approval. Members live in partially furnished apartments and may bring personal belongings to create a comfortable living environment while working toward their recovery goals.
