Located in Wilkes Barre, PA, the Fresh Food Farmacy is dedicated to providing fresh, nutritious food and essential health education to individuals facing food insecurity and health challenges. Our mission is to empower participants to take control of their health and manage medical conditions through informed dietary choices and lifestyle changes. We serve patients diagnosed with elevated HBA1C levels who receive a referral from their primary care physician. Upon enrollment, our program offers a comprehensive support system that includes clinical care management, diabetes education, and guidance from dietitians and pharmacists. Each week, participants receive a generous supply of wholesome food, enough to create balanced meals for their families twice daily over five days, ensuring access to healthy nutrition while fostering a deeper understanding of healthy living. We are committed to prioritizing the well-being of our community, and while we currently have paused new enrollments and facility tours to protect against COVID-19, our determination to support individuals in need remains steadfast.
