Cancer Transitions is a comprehensive, evidence-based program located in Redfield, SD, designed specifically for cancer survivors who have completed their treatment. We welcome individuals who have finished their treatment within the last 24 months, though our supportive services can benefit survivors at any stage of their journey. Our program focuses on enhancing the quality of life for survivors through essential resources, including nutritional support to promote healing, customized exercise plans tailored to individual needs, emotional health education and support to foster resilience, and medical management education to navigate ongoing health concerns. Our mission is to empower survivors with the knowledge and tools they need for a healthier future.
