Located in Salem, SD, the Living with Type 2 Diabetes Program is dedicated to supporting individuals who have recently received a type 2 diabetes diagnosis. This free, comprehensive program spans 12 months and provides essential resources for understanding and managing diabetes effectively. Participants will benefit from six informative e-booklets tailored to their individual needs, alongside a monthly e-newsletter filled with seasonal tips and local community event information, available in both English and Spanish. Enrollees who provide their phone number can also take part in our monthly Ask the Experts Q&A series, where they can directly engage with American Diabetes Association experts to ask questions, learn about the connection between diabetes and heart health, and receive helpful giveaways designed to enhance their journey towards better health with diabetes management.
