OPTIONS for Community Living in Dickson, TN, is dedicated to empowering the elderly and adults with disabilities who may not qualify for the CHOICES program. Our mission is to offer comprehensive home- and community-based service options that prioritize the dignity and well-being of our clients. We provide essential services such as nutritious home-delivered meals, personal care assistance, homemaker services, and case management to ensure that individuals receive the support they need. Our process begins with a compassionate Intake Specialist who will evaluate eligibility through an Intake Form. If qualified, clients will be visited by our experienced case management team to assess their needs and coordinate services. Importantly, there are no income eligibility requirements to access our programs; services are available based on a sliding fee scale.
