Project Transition provides comprehensive care for adults aged 18-65 facing mental health challenges, including those with co-occurring substance use disorders, intellectual developmental disorders, or physical health conditions. Their Tennessee locations offer both residential and day treatment programs in apartment settings. At the core of their approach is a unique emphasis on therapeutic community, referring to participants as "members" rather than clients or patients to foster a sense of belonging and community integration. This community-based model works to reduce stigma while maintaining normal social expectations. The program implements dialectical behavior therapy (DBT) with an individualized, strength-based focus in their residential mental health programs. Members receive support from a dedicated team of approximately 30 professionals, including certified peer and recovery specialists who provide daily assistance from 7 a.m. to 9 p.m., with emergency support available after hours. Daily programming includes skills workshops, medication management, individual therapy sessions, group therapy, community meetings, goal planning, and access to AA and NA meetings. Transportation is provided for off-site appointments and recovery meetings. Project Transition also offers systems of support (SOS) for children and adults whose behavioral health concerns impact their community functioning, collaborating with crisis teams and family support systems to reduce hospitalizations and prevent crises. The program measures success through 13 specific criteria and provides partially furnished apartments where members can bring personal belongings. Family visits are permitted during daytime hours with housemate approval.
