OPTIONS for Community Living, located in Seymour, TN, is dedicated to enhancing the lives of the elderly and adults with disabilities who do not qualify for the CHOICES program. Our mission is to empower individuals by providing essential home- and community-based services that include home-delivered nutritious meals, personal care, homemaker services, and comprehensive case management tailored to meet specific needs. We ensure a warm, supportive environment where each person's unique circumstances are acknowledged during the intake process. Our knowledgeable Intake Specialist uses a detailed form to assess eligibility, and if qualified, individuals receive an in-home visit from our experienced case management staff to further evaluate needs and organize necessary services. While there is no income eligibility requirement for our program, we do offer a sliding fee scale based on income to foster accessibility for everyone in our community.
