OPTIONS for Community Living is dedicated to enhancing the lives of the elderly and adults with disabilities in Signal Mountain, TN, through comprehensive home- and community-based services. Our program serves those who do not qualify for the CHOICES program, offering essential support such as home-delivered nutritious meals, personal care, homemaker services, and case management tailored to individual needs. We believe in empowering our clients by providing them with the options they deserve for a better quality of life. Our Intake Specialist facilitates the application process, utilizing an Intake Form to assess eligibility. If eligible, our case management team will conduct an in-home assessment to establish personalized services. While there are no strict income eligibility requirements, we do utilize a sliding fee scale based on income, ensuring that our services are accessible to those who need them.
