OPTIONS for Community Living in Tullahoma, TN, is dedicated to empowering our elderly community and adults with disabilities by offering essential home- and community-based services. Our state-funded program provides a wide range of services, including home-delivered nutritious meals, personal care, homemaker assistance, and comprehensive case management. We use an Intake Form to assess eligibility, and once approved, a case manager will conduct an in-home visit to set up the necessary services.
