OPTIONS for Community Living, based in Union City, TN, is a dedicated state-funded initiative designed to serve elderly individuals and adults with disabilities who do not qualify for the CHOICES program. Our mission is to empower individuals by offering home- and community-based service choices that enhance their quality of life. We provide a range of essential services, including home-delivered nutritious meals, personal care, homemaker services, and comprehensive case management, ensuring that everyone receives the support they need. Individuals seeking assistance can easily connect with our Intake Specialist, who uses an Intake Form to assess eligibility. Once determined eligible, a case manager will conduct an in-home visit to tailor services to meet individual needs. While there are no income eligibility requirements, we do have a sliding fee scale based on income, making our services accessible to those in need in our community.
