The Children with Special Health Needs (CSHN) program located in Cambridge, Vermont, is dedicated to supporting children and youth from birth to age 21 who have unique health requirements. Our mission is to ensure that these young individuals receive comprehensive, culturally sensitive, and family-centered services tailored to their needs. We provide a team of experienced medical social workers, nurses, and specialty providers who assist families in navigating the often complex health system. Our offerings include care coordination, children’s personal care services, pediatric hi-tech nursing, pediatric palliative care, child development clinics, and community nutrition services, all designed to foster health and well-being in a supportive environment.
