Cancer Transitions is a compassionate program based in Charlotte, VT, designed to support cancer survivors of any diagnosis who have recently completed their treatment, ideally within the last 24 months. Recognizing the ongoing journey of recovery, we aim to empower individuals at any stage of survivorship. Our comprehensive services include nutritional support tailored to individual needs, customized exercise programs, education on emotional well-being, and guidance in medical management.
