Located in East Montpelier, VT, Cancer Transitions is a comprehensive, evidence-based program designed for cancer survivors who have recently completed treatment. Targeted primarily at those who finished their treatment within the last 24 months, our supportive services extend to anyone in their survivorship journey. We focus on providing vital nutritional support, personalized exercise programs, emotional health education, and essential medical management resources. Our mission is to empower survivors with the knowledge and tools they need to thrive after cancer, ensuring that every individual feels supported and equipped as they navigate their recovery.
