Cancer Transitions is a compassionate and evidence-based program located in Fairfax, VT, dedicated to supporting cancer survivors who have recently completed their treatment. Designed primarily for those who have finished treatment within the last 24 months, our services are beneficial at any stage of survivorship. We focus on holistic well-being by offering nutritional support, personalized exercise plans, emotional health education, and insights into medical management.
